UNACCOMPANIED MINORS

 Parental Permission Form - Any person who has not yet reached the age of 18 who is not being accompanied to Calumet by his or her own parent or legal guardian must have a completed form on file at the Calumet office prior to or upon arrival. A separate form must be provided for each person under 18 years of age.

 Transfer of Responsibility - Use this form to grant permission for your child to come to Calumet under the supervision of another family, adult, or guardian over the age of 18.
